We cannot process any requests without a letter of representation on file. The letter of representation must meet the requirements listed below

The Department is mandated to follow strict guidelines before releasing information to a custodial or non-custodial parent's attorney. The attorney representing a custodial parent or non-custodial parent must submit an individual request in writing printed on their letterhead.

The letter must meet the following requirements:

Indicate the client you are representing, including a case identifier.
State specific information being requested.
Call back and fax number.
The letter must be signed by the attorney.

We cannot honor a one-time generic request or a request made on a fax cover sheet. Please resubmit your request following the guidelines the Department is mandated to use.

Due to strict laws and Department policy regarding confidentiality, we are prohibited from providing case information to a third party without the express written and notarized authorization of the custodial or non-custodial parent.

You may fax your request or inquiry on your letterhead to 217-524-4517.  It will be handled as appropriate based on the request.
